The role includes crisis planning, plan implementation monitoring, and case management with family training. A Bachelor's in a related field is required; 2+ years of relevant experience, travel, and valid credentials are expected.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education.
• Master’s degree is a plus.
• Minimum 2 years experience with persons with intellectual and/or developmental disabilities.
• 1-year experience if paired with Master’s degree.
• Must meet requirements for both Qualified Developmental Disability Professional and Qualified Mental Health Professional.
• Direct support experience supporting individuals with mental health and developmental disability diagnoses.
• Experience with crisis management and crisis counseling.
• Valid driver’s license.

The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, planned respite and emergency respite.Salary: $56,000/AnnuallyResponsibilities:Develops and revises the cross systems crisis plans in a timely fashion.Monitors the implementation of the plan.Performs Case Management functions of linking and arranging for services and referrals.Provides families and stake holders with individual-specific training and training in the knowledge, skills, and abilities required by the population and age to be served.Provides consultation to the system on the clinical needs of the individual's care.Requirements:Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plusMinimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degreeMust meet requirements for both Qualified Developmental Disability Professional and Qualified Mental Health Professional.
