Job summary

Qualifications

Responsibilities

• Community & Connection: Plan activities and engage residents in a joyful home environment.
• Reporting: Complete daily progress notes and documentation with accuracy.

Key Responsibilities

• Daily Living Assistance: Support participants with ADLs, including meal preparation, personal hygiene, and light housekeeping to keep the home running smoothly.
• Medication & Safety: Administer prescribed medications accurately and prioritize the health and welfare of every resident.
• Goal Mentorship: Actively engage with participants to help them achieve their personal goals and skill-building milestones.
• Community & Connection: Plan activities, go out—most importantly—have fun with the residents!
• Reporting: Complete daily progress notes and documentation with accuracy and care.
