Advocate Aurora Health is seeking a Registered Nurse (RN) in Home Health Case Management for a nursing job in Oak Forest, Illinois.

Position: Registered Nurse - Home Health Case Manager

Location: Far Southwest Suburbs, focusing on Worth, Alsip, Merrionette Park, and surrounding communities

Hours: Days with weekend rotation

This position is eligible for a sign-on bonus of up to $10,000! You must be working in this same position at the time of payout. Current internal team members are not eligible.

Major Responsibilities:
  • Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum, demonstrating adaptive communication, providing individualized care, involving patients and families in planning, and adopting a holistic approach.
  • Ensuring/Improving Quality Care: Facilitates quality, cost-effective care, mobilizes resources, adheres to standards, participates in quality initiatives, and ensures safety and regulatory compliance.
  • Patient/Staff Education: Meets learning needs of patients and families, utilizes resources, and promotes educational growth among staff.
  • Nursing Process: Uses nursing process for problem-solving, recognizes clinical changes, makes clinical decisions, prioritizes care, coordinates referrals and discharges, and applies ethical practices.
  • Leadership/Collaboration/Professional Development: Collaborates effectively, demonstrates leadership, participates in meetings, maintains knowledge, and pursues certifications or degrees.
Licensure:
  • Registered Nurse (RN) license in Illinois
Education/Experience:
  • Graduate from an accredited nursing program
  • Minimum 10 continuing education credits annually
  • At least one year of medical-surgical experience
Knowledge, Skills & Abilities:
  • Strong communication and interpersonal skills
  • Good documentation and computer skills
  • Time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license and CPR certification
Physical Requirements & Working Conditions:
  • Ability to work weekends, holidays, and various shifts
  • On-call availability
  • Ability to handle multiple tasks and transport patients/equipment
  • Work in diverse environments, including patient homes
  • Ability to lift up to 35 pounds without assistance; patient handling equipment used for heavier lifts
