Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Glenwood, Illinois.

Major Responsibilities include:

• Developing and maintaining therapeutic nurse/patient relationships across the healthcare continuum.
• Ensuring quality, cost-effective care by mobilizing resources, participating in quality initiatives, and adhering to standards.
• Meeting patient and family educational needs, assessing readiness, and promoting learning.
• Applying the nursing process for clinical decision-making, prioritizing care, and coordinating discharge planning.

Licensure required: Valid Illinois RN license and a valid driver's license. Education and experience include graduation from an accredited nursing program, one year of medical-surgical experience, and ongoing continuing education credits.

Physical requirements include the ability to work flexible hours, handle physical tasks such as lifting up to 100 pounds, and travel within the service area in various conditions.
