Starting pay $36.90. Pay will commensurate with experience. Night Shift Differential $4.90/HR.

Sign on Bonus of $3,500, half paid on first check, half after 18 months (External Applicants Only). The bonus is prorated based on FTE.

Offering both full time and part time roles.

Flexible schedule; shifts can be 11p-7a or 8p-8a.

Job Description Summary

Provides the direct continuous skilled nursing care of acute, chronically ill, and terminally ill patients in the home setting as prescribed by NCH and community physicians, in compliance with regulatory bodies. Provides developmentally appropriate care that reflects the highest standards of nursing practice and in a manner which promotes quality, continuity, and safe patient care. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses and maintains a current Ohio Nursing License.

Job Description

Essential Functions

• Provides nursing care using a family-centered approach.


• Assesses individualized patient needs, communicates patient care needs, priorities, updates, and changes in condition to the assigned Home Health Nurse Supervisor or Manager and, as needed, to the Staffing Specialists per the policies and guidelines.


• Utilizes therapeutic communication while monitoring the physical, psychological, and social needs of patients and families.


• Engages in the workplace community by staying aware of changes, participating in data collection or auditing, and understanding quality improvement.


• Collaborates to develop an individualized care plan using standards, protocols, and current nursing literature and research. Implements and follows the individualized plan of care for each patient, which states the patient outcomes, goals, and means by which these goals will be achieved.

• ADN, required.


• BSN required within 5 years from date of hire.

Licensure Requirement

• Ohio RN license, required.
