The Staff RN-Ambulatory Clinics works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family-centered care. The Staff RN-Ambulatory Clinics systematically and continuously collects and assesses data related to patient health status to develop, execute, and evaluate the plan of care. The Staff RN-Ambulatory Clinics utilizes evidence to drive clinical decisions and practice related to individual and population health. The Staff RN-Ambulatory Clinics establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Staff RN-Ambulatory Clinics considers factors related to safety, effectiveness, and fiscal accountability in planning and delivering care. The Staff RN-Ambulatory Clinics delegates and supervises care in accordance with the WI Nurse Practice Act. The Staff RN-Ambulatory Clinics participates in continuous quality improvement initiatives and assumes responsibility for their professional development. Major responsibilities include patient care coordination, direct patient care, patient education, telephone triage/eVisits, and assisting with in-clinic minor procedures. May float to other areas as needed. Other duties as assigned.
Clinical experience and/or interest in the care and management of patient populations specific to the area. At least 1 year of relevant nursing experience is preferred.
Required: Nursing licensure. Preferred: Bachelor's Degree in Nursing.
Requires current Wisconsin RN license or multi-state license from a participating NLC state. BLS certification from AHA or equivalent within 90 days of hire.
