Responsibilities

• Assess and develop care plans for home health patients
• Ensure activities align with the organization's mission
• Maintain patient confidentiality per HIPAA regulations

Minimum Requirements

• Current, unrestricted state RN license
• At least 1 year of clinical care experience as a nurse
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, and auto insurance
• Associates or Bachelor’s Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred
