Advocate Aurora Health is seeking a Registered Nurse (RN) for a Home Health position in Westmont, Illinois.

Additional Position: Registered Nurse (RN) Part Time - Home Care Liaison

Location: Good Samaritan Hospital, Downers Grove

Hours: Part-time days with weekend and holiday rotation. Schedule typically 2 days one week, 3 days the next.

Major Responsibilities:

1. Screening and accurate completion of referrals to ensure patients meet admission criteria and comply with regulations, including processing referrals from hospital sites and MD offices, verifying benefits, obtaining authorizations, and ensuring physician orders are obtained.
2. Providing quality customer service, promoting collaboration between referral sources and home care services, educating teams on services, maintaining knowledge of reimbursement guidelines, and ensuring proper follow-up and patient satisfaction.
3. Ensuring complete referral information for accurate billing, collaborating with insurance verification, and educating physicians on Face-to-Face requirements.
4. Maintaining clinical skills, participating in team meetings, suggesting process improvements, and participating in training and orientation.

Licensure:

• Registered Nurse (RN)

Education/Experience:

• RN with 3-4 years’ experience; 1-2 years in home care preferred

Knowledge, Skills & Abilities:

• Excellent communication, knowledge of Medicare rules, multitasking skills, customer service, managed care knowledge, critical thinking, current RN licensure.

Physical Requirements:

• On-call and weekend rotation, ability to work in a fast-paced environment, ability to drive in all weather conditions, lift up to 35 pounds, use patient handling equipment as needed.
