Base pay range
$60,000.00/yr - $100,000.00/yr
Summary: The Integrated Care Nurse acts as a liaison between primary care and behavioral health providers, ensuring patient care integration and facilitating communication between providers and partner agency care teams (i.e. nurses, social workers). The Integrated Care Nurse assists with direction of daily patient visits, assessment and triage, care coordination, and health education and information. The Integrated Care Nurse works collaboratively to assure the maintenance of a safe patient care environment, safe transition to care both within Tapestry 360 Health and with other community agencies and contributes to quality assurance and other organizational activities as needed. The Integrated Care Nurse provides patient-centered, trauma-informed services to patients with severe mental illness and co-occurring substance abuse/addiction disorders.
Essential Duties And Responsibilities
- Provides phone and walk in triage, facilitates medication refills,andadministers medications.
- Promotes health and well-being for patients and families by assessing health literacy, providing meaningful health information, and supporting self-management of chronic physical and mental illness.
- Supports Medical Assistants by providing information, ongoing development and training.
- Assures safe patient care environment. Oversees infection control practices and management of the supply and storage of medication.
- Performs clinical patient visits such as administering long-acting psychotropic injections, providing education and/or follow-up on diabetes care, asthma and/or other chronic diseases.
- Collaborates with care team members to support patients during transitions of care:
- Provides necessary support in the coordination of referrals, including the access of necessary tests or other values and reports as needed
- Provides information and support to patients and families and works with the care team to assure appropriate follow up.
- Fosters relationships with community agencies, recovery counselors as needed.
- Additional responsibilities may includ e serving as the electronic medical record superuser and/or providing group health education.
- Participates in required care team and other organizational meetings.

Education And/or Experience
- Active Illinois Registered Nurse License and CPR certification required prior to hire date.
- Bachelor’s degree or higher preferred.
- Behavioral health experience required.
- Experience in community health, health education, care coordination and disease management preferred.
Communication Skills
- Bilingual Spanish/English preferred.
- Excellent verbal and written communication skills required.
Analytical Skills
- Ability to read and interpret documents.
Mathematical Skills
- Ability to add and subtract two-digit numbers and multiply and divide with 10s and 100s.
Problem Solving
- Ability to define problems, collect data, establish facts, and draw valid conclusions.

This range represents the potential compensation for the role based on experience, tenure, and performance over time.
Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
