Advocate Aurora Health is seeking a Registered Nurse (RN) for a Home Health nursing job in Random Lake, Wisconsin.
Job Description & Requirements
Major Responsibilities:
Performs all scheduled visits and collaborates with the interdisciplinary team as necessary.
Conducts thorough physical, psychosocial, and environmental assessments of patients and updates the care plan accordingly.
Communicates with physicians to confirm and update the patient's treatment plan and relays changes to the RN Case Manager.
Provides ongoing assessment of patient responses to treatments, medications, and education, and updates the care plan as needed.
Partners with the RN Case Manager for discharge planning, utilizing community and interagency resources to ensure continuity of care.
Documents and updates patient records in accordance with guidelines.
Communicates problems and plans clearly to physicians and RN Case Manager.
Follows policies and monitors updates.
Performs patient positioning and transporting duties, requiring proper technique.
Must demonstrate knowledge and skills to provide age-appropriate care, understanding growth and development across the lifespan.
Licensure, Registration, and/or Certification Required:
Education Required:
Completion of an accredited Nursing program
Experience Required:
Typically 1 year of nursing experience
Knowledge, Skills & Abilities:
Proficiency in clinical skills, good communication, ability to develop rapport, make independent decisions, and organize work effectively. Must have a valid driver’s license and a functional vehicle with insurance.
Physical Requirements and Working Conditions:
Ability to sit, stand, walk, lift, and perform physical tasks as described, including lifting up to 50 lbs, pushing/pulling, and operating necessary equipment. Must wear protective gear when exposed to hazards. Good speech, vision, hearing, and manual skills required.
