Advocate Aurora Health is seeking a Registered Nurse (RN) for a Home Health nursing position in Cascade, Wisconsin.

Job Description & Requirements

Major Responsibilities:

• Perform scheduled visits and collaborate with the interdisciplinary team.
• Conduct physical, psychosocial, and environmental assessments; update care plans accordingly.
• Communicate with physicians to update treatment plans and inform RN Case Manager of changes.
• Assess patient responses, administer treatments, and revise care plans as needed.
• Partner with RN Case Manager for discharge planning and utilize community resources to ensure continuity of care.
• Document and update patient records per guidelines.
• Communicate problems and plans clearly to physicians and RN Case Manager.
• Follow policies and monitor updates.
• Assist with patient positioning and transport, utilizing proper techniques.
• Provide age-appropriate care, demonstrating knowledge of growth and development principles.

Licensure, Registration, and Certification:

• Valid RN license in the practicing state.
• Basic Life Support (BLS) certification from AHA.
• Driver's license from Wisconsin or Illinois.

Education and Experience:

• Completion of an accredited nursing program.
• Typically 1 year of nursing experience.

Knowledge, Skills & Abilities:

• Proficiency in clinical skills, sound judgment.
• Excellent communication and interpersonal skills.
• Ability to develop rapport with patients, staff, and physicians.
• Ability to make independent decisions.
• Organizational and problem-solving skills.
• Ability to multitask effectively.
• Valid driver's license and functional vehicle with insurance.

Physical Requirements and Working Conditions:

• Ability to sit, stand, walk, lift, and perform physical tasks as described.
• Lift up to 50 lbs, carry 40 lbs, push/pull 30 lbs.
• Exposure to chemicals, hazardous waste, blood, and body fluids with necessary protective gear.
• Functional speech, vision, hearing, and touch.
