Job Responsibilities

• Assess behavioral health patients and their families to identify physical, psychosocial, and environmental needs.
• Manage caseloads, including planning, coordinating, and evaluating treatment plans.
• Gather and incorporate data on physical, psychological, social, and spiritual factors affecting patient health into care plans.
• Perform initial and follow-up evaluations, and communicate significant findings to the care team.
• Adjust care plans based on patient needs and coordinate with team members.
• Educate patients, families, and facility staff on behavioral health and pain management.
• Supervise and document care provided by aides and LPNs.
• Report any facility issues and provide support during end-of-life care and bereavement.

Minimum Requirements

• Current, unrestricted RN license in the state of Ohio.
• At least 1 year of clinical nursing experience.
• Willingness and ability to travel within the service area, with a dependable vehicle, valid driver’s license, and auto insurance.
• Associates or Bachelors Degree in Nursing preferred.
• Experience in Home Health, Hospice, or with Homecare HomeBase is a plus.
