Job Overview

Elara Caring is seeking a dedicated Registered Nurse (RN) specialized in Behavioral Health for a home health nursing position in Toledo, Ohio. This is an ongoing, full-time role focused on providing high-quality, compassionate care in patients' homes.

Responsibilities
  • Assess and manage a caseload of behavioral health patients and their families, identifying physical, psychosocial, and environmental needs.
  • Develop, implement, and evaluate individualized care plans.
  • Perform initial and ongoing patient evaluations, adjusting care as needed.
  • Communicate effectively with the interdisciplinary team regarding patient progress and issues.
  • Provide education and counseling to patients and families about behavioral health management.
  • Supervise behavioral health aides and LPNs, ensuring adherence to care protocols.
  • Support patients at end-of-life and conduct bereavement assessments.
Minimum Requirements
  • Current, unrestricted RN license in the state of Ohio.
  • At least 1 year of clinical nursing experience.
  • Willingness and ability to travel within the service area (50%), with a reliable vehicle, valid driver’s license, and auto insurance.
  • Associates or Bachelors Degree in Nursing preferred; previous home health or hospice experience is a plus.
