Counterpart Health is a subsidiary of Clover Health that builds AI-enabled primary care software. Its flagship product, Counterpart Assistant, is used by thousands of practitioners to support earlier diagnosis and management of chronic conditions, reduce medical expenses, and drive success in value-based care programs.
The role
The Staff Product Manager for the Longitudinal Patient Record owns one trusted, unified view of the care each patient receives across every setting — from the primary care office to specialists to hospital stays. Clinical data flows in from hundreds of sources including health information exchanges, EHRs, labs, ADT feeds, pharmacy fills, prior authorizations, and claims. This role connects those sources into a single history of each patient's visits, conditions, medications, and results. Every Counterpart product builds on this record, and the company's AI reasons over it, so improvements here expand what AI can reliably do. You would report to the Director of Product, Clinical Data Products, and lead the Longitudinal Patient Record pod, partnering closely with engineering, the clinical team, and data consumers across the company.
What you would do
- Set the vision and roadmap for the longitudinal record, identifying the highest-value problems across the products and programs that depend on it, and making trade-offs among completeness, accuracy, timeliness, and investment.
- Define how care events and their dates, settings, providers, and facilities are represented consistently, and how medications, conditions, labs, and procedures connect to those events and to the patient's history over time.
- Build and maintain a unified record of acute events — each hospitalization, ED visit, and post-discharge care management episode — drawn from ADT notifications, prior authorization notices, facility claims, medical records, and discharge documentation.
- Ensure the record serves every use case: point-of-care providers, quality measurement (HEDIS), risk adjustment and RADV audits, payment integrity, and revenue operations.
- Own patient matching, deduplication, normalization, and clinical terminology mapping so that every new data source is turned into a trusted part of the patient record through shared, reusable processes.
- Drive adoption of canonical fields and definitions: write requirements, create migration plans per use case, and retire fragmented or duplicate versions of the same data across products.
- Work directly in the data — reviewing charts, investigating discrepancies, running analyses, and testing definitions against real records before they ship.
What they are looking for
- 8 or more years of product management experience, including ownership of a data platform, data model, or shared data product used by multiple teams.
- Experience setting product strategy for a platform with many internal consumers and making the trade-offs that come with it.
- Deep working knowledge of healthcare data and its clinical utility: institutional and professional claims, clinical documents (C-CDA), FHIR resources, ADT feeds, and lab data.
- Experience defining canonical or master data (patient, provider, facility, encounter, clinical concepts) and driving adoption across other teams.
- Comfort working directly in the data: reviewing charts, investigating discrepancies, running analyses, and validating definitions against real records; experience using AI to accelerate this work.
- A track record of leading cross-functional projects through influence, setting direction and driving delivery across engineering, clinical, and operations teams without direct management authority.
- Ability to point to a specific data improvement that contributed to a measurable business or clinical result.
