**Job Summary:**

Provides leadership in optimal product preparation and dosing of medications, and provision of patient-specific drug information, including patient-specific dosing. Supports the prevention and reporting of medication errors and adverse drug reactions according to policy. Supports quality relationships with healthcare professionals, families, and patients; and the pharmacist is actively involved in education and training in all appropriate ways.

**Key Responsibilities:**

  1. Reviews drug orders and prescriptions to ensure safe, appropriate, and cost-effective medication use.
  2. Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  3. Prepares and dispenses medications, provides drug information, and manages medication therapy in accordance with regulations and hospital policy.
  4. Supervises support personnel activities and verifies the accuracy and appropriateness of all medications prior to dispensing.
  5. Additional duties may be assigned as needed.

**Technical Capabilities:**

  1. Pharmaceutical Computer Literacy (Novice): Demonstrates understanding of pharmacy computer applications supporting medication use processes.
  2. Pharmaceutical Drug Preparation, Dispensing & Distribution (Intermediate): Proficient in drug preparation, dispensing, and distribution, including sterile preparations and training others.
  3. Medication Management (Intermediate): Capable of conducting medication management independently, including evaluating clinical appropriateness and monitoring laboratory results.
  4. Mentoring & Coaching (Novice): Able to train and mentor others, providing constructive feedback and support.
