Job summary

Polaris Pharmacy Services is hiring a Staff Pharmacist to support day-to-day dispensing for long-term and post-acute care. You will ensure accuracy of labels, maintain patient medical histories, and manage controlled substances in coordination with the pharmacy team.

The role requires a current pharmacist license, a PharmD or PharmB, and clinical collaboration with physicians, nurses, and residents to ensure safe therapy and regulatory compliance.

Qualifications

• Active Pharmacist license in good standing in the practicing state.
• Pharmacy degree (PharmD or PharmB) from an accredited school.
• Experience in acute-care settings is preferred.

The Staff Pharmacist must adhere to the company’s quality and customer service standards, policies and procedures, as well as the State and Federal guidelines.Duties/Responsibilities:Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medicationsMaintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugsWork with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulationsProvide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customersDocument all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical recordPreparation and/or supervision of all sterile IV drug products or solutionsProvide evening, weekend, and holiday coverage during rotation and as required by managementFollow all applicable government regulations including HIPAAOther duties as assigned; Job duties may vary depending on business needsResponsible for the flow of work and appropriate outcomesQUALIFICATIONS/COMPETENCIES:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
