Job summary

Polaris Pharmacy Services seeks a Staff Pharmacist to handle daily dispensing and ensure quality and regulatory compliance in a long-term/post-acute care setting. You will verify labels, manage patient data, and support IV compounding under state and federal guidelines.

The role requires strong communication, precise dispensing, and flexibility to cover evenings, weekends, and holidays. A PharmD or BSP/PharmD with active state licensure is essential, plus acute-care experience.

Qualifications

  • Pharmacist must be licensed in the state of practice.
  • Minimum 2 years of acute-care experience or 1 year as a production pharmacist.

Responsibilities

  • Verify completeness and accuracy of all new and refill labels and orders.
  • Maintain resident medical history and data, clinical and laboratory data, and concurrent medications.
  • Assist in ordering, handling, receiving, documenting, and storing controlled substances; maintain a perpetual inventory for schedule II drugs.
  • Provide clinical expertise by responding to drug-related questions from staff and patients.
  • Document all communications with physicians, nurses, and other healthcare team members in the medical record.
  • Prepare and supervise sterile IV drug products or solutions.
  • Provide evening, weekend, and holiday coverage as required by management.
  • Follow HIPAA and government regulations; other duties as assigned.

The Staff Pharmacist must adhere to the company’s quality and customer service standards, policies and procedures, as well as the State and Federal guidelines.SCHEDULE: 9:00am - 7:30am (Four days/week, 10 hour shifts)Duties/Responsibilities:Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medicationsMaintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugsWork with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulationsProvide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customersDocument all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical recordPreparation and/or supervision of all sterile IV drug products or solutionsProvide evening, weekend, and holiday coverage during rotation and as required by managementFollow all applicable government regulations including HIPAAOther duties as assigned; Job duties may vary depending on business needsResponsible for the flow of work and appropriate outcomesQUALIFICATIONS/COMPETENCIES:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
