Job summary

Fort Defiance Indian Hospital Board, Inc. is actively recruiting Pharmacist-CACs to support its rural healthcare mission. The Clinical Applications Coordinator will lead and train on medication use systems and CDS tools within the EHR and pharmacy environment at FDIHB.

Qualifications include 5+ years in pharmacy with 3+ in hospital settings, a Pharm.D. or Bachelor’s degree in Pharmacy, and an active, unrestricted license in any U.S. state.

Qualifications

• Five or more years of experience in pharmacy practice, including three years in a hospital setting.
• Completion of an accredited college or university with a Bachelor’s degree in Pharmacy or Pharm.D.
• Active, unrestricted Pharmacy License in any U.S. State or Territory, maintained by completing the state Board of Pharmacy requirements.

Duties

The Clinical Applications Coordinator will lead, recommend, develop, implement, manage, and train on medication use systems and other clinical decision support tools within the electronic health record (EHR) and pharmacy systems. Responsibilities include customizing, evaluating, configuring, analyzing, and documenting the EHR system and business workflows as well as providing technical training and support to all hospital staff.

Qualifications

• Five (5) or more years of experience in pharmacy practice, including three (3) years in a hospital setting.
• Completion of an accredited college or university with a Bachelor’s degree in Pharmacy or Pharm.D.
• Active, unrestricted Pharmacy License in any U.S. State or Territory, maintained by completing the state Board of Pharmacy requirements.
