Job summary

Third Street Family Health Services is seeking a Staff Pharmacist to ensure safe dispensing, provide patient counseling, and collaborate with care teams across our community health centers.

You will lead pharmacy staff, manage medication safety, and help ensure compliant operations while promoting patient-centered care in a mission-driven setting.

Qualifications

• Bachelor in Pharmacy; Pharm.D preferred.
• Ohio Board of Pharmacy license.
• Board Certification in Pharmaceutical Care preferred.
• Immunization training required.
• 3 years’ experience as a Pharmacist.
• Current state license(s) and controlled substance license to practice in Ohio; maintain credentialing status.

Responsibilities

• Ensure safe, accurate, and compliant dispensing and pharmacy operations.
• Counsel patients and caregivers and support positive medication outcomes.
• Collaborate with providers on prescriptions, alternative therapies, and prior authorizations.
• Support compliance with federal, state, and 340B requirements.
• Maintain effective medication inventory, procurement, and security processes.
• Provide clinical guidance and day-to-day leadership to pharmacy staff.
• Coach and develop team members while promoting quality and accountability.
• Help create a safe, welcoming pharmacy experience for every patient.
• Perform other duties as assigned.

Qualifications:

• Ohio Board of Pharmacy Registered Pharmacist (RPh) License
• Board Certification in Pharmaceutical Care preferred;
• Immunization Training required
• Experience: 3 years’ experience as a Pharmacist;
• Certificate/License: Current state license(s) and controlled substance license to practice pharmacy in the State of Ohio. Ability to maintain complaint credentialing status
