Qualifications

• One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.

Responsibilities

• Conduct a thorough patient profile review to assess appropriateness of prescribed medication, including monitoring for duplications, interactions, allergies, dose, strength, and route.
• Promote rational, cost-effective drug therapy by formulary optimization and appropriate treatment options.
• Evaluate provider orders and verify correctness in the pharmacy system and patient profiles.
• Direct pharmacy technicians in all areas of technical responsibilities.
• Ensure proper preparation, labeling, and documentation of medications.
• Ensure proper storage, dispensing, recording and inventory of controlled substances.
• Interview patients and provide education regarding medications.
• Ensure proper preparation, labeling, and documentation of bulk packaged items.
• Ensure aseptic preparation, labeling and delivery of compounded sterile products with proper garbing, handwashing and aseptic technique.
• Provide drug information to patients and healthcare staff; conduct in-services as requested.

The UW Medical Center-Montlake Pharmacy Department has an outstanding opportunity for a Staff Pharmacist - Ambulatory Pharmacy (Pharmacist 2).

WORK SCHEDULE

POSITION HIGHLIGHTS

• Perform professional pharmaceutical dispensing and clinical functions in the Ambulatory Pharmacies.
• This position provides drug information and education to patients and family members, providers, nurses and other health-care professionals, and provides pharmaceutical care to patients.

PRIMARY JOB RESPONSIBILITIES

• Conduct a thorough patient profile review to assess the appropriateness of the prescribed medication including monitoring for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in medical record as required.
• Promote rational, cost-effective drug therapy by promoting the use of formulary items and optimizing patient care through appropriate selection of treatment options.
• Evaluate all provider orders and verify correctness of entry in the pharmacy computer system and computerized patient profiles.
• Direct pharmacy technicians in all areas of technical responsibilities.
• Ensure proper preparation and labeling of all medications.
• Ensure that controlled substances are properly stored, dispensed, recorded, and inventoried.
• Interview patients and provide education regarding their medications.
• Ensure that all bulk packaged items are properly prepared, labeled and documented.
• Ensure correct aseptic preparation, labeling and delivery of compounded sterile products. Perform routine visual observation and oversight to ensure proper garbing, handwashing, cleaning and aseptic technique processes is followed during sterile compounding.
• Provide drug information to patients, physicians, nurses, and other members of the health care team. Provide in-services to staff as requested.

REQUIRED POSITION QUALIFICATIONS

One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
