Job summary

Saint Vincent Hospital in Worcester, MA is seeking a pharmacist to provide pharmaceutical care across the patient care team, including oversight, monitoring, preparation, dispensing, and documentation of therapy.

You will ensure compliance with Federal, State, and local laws, advise on medication utilization, and contribute to a multidisciplinary environment. Hospital experience and licensure are required or preferred.

Qualifications

  • Pharmacist degree and license required.
  • BLS within 30 days of hire; ACLS/PALS if code attendance required.
  • Hospital pharmacy experience preferred.

Responsibilities

  • Review and monitor patient's medication therapy for safety and efficacy; advise care team on therapies.
  • Order verification and medication profile reviews for interactions, dosing and duplications.
  • Prepare, label and dispense medications per physician prescriptions; maintain required records.
  • Check high risk IVs, renal dose adjustments, IV to PO conversions, and box warning interventions.
  • Ensure medications are available, properly procured, stored and maintained.
  • Performs related duties as required.

Job Summary

Provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Additionally: ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization.


Responsibilities


  • Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions.

  • Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.

  • Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records

  • Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions.

  • Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained.

  • Performs related duties as required
