Saint Vincent Hospital in Worcester, MA is seeking a licensed social worker to join our behavioral health team. You will assess patients, develop and monitor treatment plans, and perform psychosocial assessments, ensuring documentation complies with laws and regulations.

The role includes crisis evaluations, group and individual therapy, discharge planning, and collaboration with a multidisciplinary team. Requirements: Master's degree in Social Work, active licensure, CPR, and at least two years

Qualifications

• Master's degree in Social Work or related field is required.
• 2 years of related experience.
• Currently licensed, certified, or registered to practice as required by state law.
• CPR certification is required.

Responsibilities

• Must be able to perform crisis evaluations and discharge planning.
• Performs a mini-mental status exam and psychosocial assessment on all new patients.
• Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines.
• Performs related duties as required
• Additional responsibilities include responding to patient needs and coordinating with treatment teams.

Job Summary

Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.
