Job Summary
The Partial Hospital Program staff clinician in mental health is a Masters prepared clinician who has clinical responsibilities relatedQualifications
Job Specific Responsibilities; (e.g. clerical, patient care, food service, supervisory, etc.)
• Completes Psychosocial Assessments and Treatment Plans
• Completes documentation according to policy
• Facilitates individual psychotherapy
Facilitates psychotherapeutic and psychoeducational groups
• Coordinates aftercare plans to ensure continuous care after discharge
• Communicates effectively with representatives of contracted Health Plans to ensure compliance with plan requirements, including the timely provision of required data.
• May provide UR and obtain insurance authorization with health plans as needed.
• Conducts individual and family therapy sessions and facilitates meetings with outside providers as needed.
• Meets with each patient and/or family on a daily basis and documents interaction in medical record.
• Completes After Visit Summary at the end of the episode of care.
• Maintains strict patient confidentiality.
• Participates in department meeting and Medical Group level meeting as assigned.
• Demonstrates abilities and effectiveness in leadership of case management program activities with assigned personnel, peers and customers.
• Demonstrates skills for providing program related education to colleagues.
• Demonstrates skills and knowledge for training and orientation of assigned personnel.
• Maintains professional growth and development through seminars, workshops and professional affiliations, integrates new knowledge into daily practice.
• Demonstrates ability to supervise, give input into annual evaluations, and disciplinary actions of technical staff.
• May be asked to provide coverage on the inpatient units as needed and dictated by census
