Job description

The Partial Hospital Program staff clinician in mental health is a Masters prepared clinician responsible for the treatment of adolescent patients in the PHP. This includes assessment, diagnosis, case management, discharge planning, group therapy, family, and individual treatment of patients admitted to the program. The person in this position also complies with all governmental and other regulatory requirements for documentation and care standards. Reports directly to the PHP clinical manager.

Job Specific Responsibilities:

1. Completes Psychosocial Assessments and Treatment Plans.
2. Completes documentation according to policy.
3. Facilitates individual psychotherapy.
4. Facilitates psychotherapeutic and psychoeducational groups.
5. Coordinates aftercare plans to ensure continuous care after discharge.
6. Communicates effectively with representatives of contracted Health Plans to ensure compliance with plan requirements, including the timely provision of required data.
7. May provide Utilization Review (UR) and obtain insurance authorization with health plans as needed.
8. Conducts individual and family therapy sessions and facilitates meetings with outside providers as needed.
9. Meets with each patient and/or family daily and documents interactions in the medical record.
10. Completes After Visit Summary at the end of each episode of care.
11. Maintains strict patient confidentiality.
12. Participates in department and Medical Group level meetings as assigned.
13. Demonstrates leadership in case management activities with assigned personnel, peers, and customers.
14. Provides program-related education to colleagues.
15. Trains and orients assigned personnel.
16. Maintains professional growth through seminars, workshops, and affiliations, integrating new knowledge into daily practice.
17. Supervises and evaluates technical staff, providing input into annual reviews and disciplinary actions.
18. May provide coverage on inpatient units as needed based on census.
