Job summary

UnityPoint Health is seeking two Senior Managers of Care Management to lead day-to-day operations across multiple markets within IA, IL, and WI. You will direct staffing, scheduling, and budget management for care management RNs, social workers, and community health workers, ensuring quality, patient experience, and financial outcomes.

The role requires a BSN with 5+ years of leadership in care management; an MS in Nursing or Healthcare Management is preferred.

Qualifications

• Bachelor's degree in Nursing (BSN) is required.
• Master's degree in Nursing or Healthcare Management is preferred.
• Minimum 5 years of leadership experience in care management or healthcare management.
• Experience in personnel management, budgeting, and financial management preferred.
• RN license required; required state trainings within specified timelines.

Responsibilities

• Leads staffing and workflows of care management RNs, Social Workers and CHWs; oversees scheduling, revenue cycle and budget management, compliance and risk, clinical and operational metrics.
• Monitors all operational components and drives continuous improvement to meet org objectives.
• Partners with clinical and administrative leaders to coordinate services, align resources, implement policies, and support growth across sites.
• Collaborates with operations and physician leadership to deliver effective and efficient care management services to defined populations.
• Serves as a resource across departments for assistance in resolution and/or clarification of issues involving care management.

• At least 4 years of clinical management or equivalent healthcare management experience is required
• Experience in personnel management, financial management, budgeting, and computer proficiency is preferred
• Care Management or Case Management experience is preferred

Licenses/Certifications:

• RN license required
• Child Abuse Mandatory Reporter Training in state(s) where providing care is requried within 60 days of hire
• Adult Dependent Abuse Mandatory Reporter Training in state(s) where providing care is required within 60 days of hire
