Qualifications

• Bachelor's degree or higher required.
• 1+ years of Care Management experience.
• 1+ years of experience in Sunrise Patient Financials Analytics or Clinical Quality.
• RN/LCSW licensure in IA or LICSW/LISW/LCSW options considered.

Responsibilities

• Provide strategic leadership and operational oversight for Integrated Care Management programs across hospital and ambulatory settings.
• Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers.
• Build strong partnerships with clinical leaders, community organizations, and cross-functional stakeholders.
• Drive process improvement initiatives to reduce readmissions, improve patient flow, optimize length of stay, and enhance care coordination outcomes.
• Ensure compliance with federal and state regulatory requirements.
• Coach, mentor, and develop team members while fostering collaboration, accountability, and service excellence.
• Analyze clinical, operational, and financial performance metrics to support strategic growth and quality improvement.

This leadership role provides the opportunity to guide care management teams in delivering patient‑centered care across both hospital and ambulatory settings. Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs.

The estimated hourly rate for this position is $120,000.00
