Job summary

HeartShare is seeking a Sr Day Support Professional to lead plan implementation and community activities, ensuring each individual’s goals and outcomes are met, while safeguarding overall well-being.

Responsibilities include staff training, plan reviews, incident reporting, and coordination with family and care providers. The role requires AMAP/CPR/First Aid certifications and a strong focus on person-centered, community-based services.

Qualifications

  • Minimum 2 years in the field with experience in supervision and paperwork.
  • Minimum of 2 years or more experience working with individuals with developmental disabilities.
  • Prior staff supervision experience strongly preferred.
  • AMAP CPR/First Aid and SCIP certifications may be required.

Responsibilities

  • Oversee DSP implementation of individuals’ plans and community activities.
  • Promote choice and self-determination in services and goals.
  • Ensure DSPs support individuals according to services and outcomes; engage in skills development.
  • Promote community inclusion and foster friendships.
  • Maintain communications with family and advocates and act as advocate.
  • Transport individuals using agency vehicles or public transport.
  • Plan monthly schedule for community outings and in-house activities.
  • Participate in program planning meetings.
  • Create staff action plans and submit for supervisor approval in Precision Care.
  • Review notes and life plans to ensure goals are consistent.
  • Send copies of staff action plans to care manager, residence, and family.
  • Upload meeting documents into Precision Care and train staff on plans.
  • Document data in medical, behavioral, and other areas; ensure accuracy.
  • Complete monthly summaries by the 10th of every month.
  • Ensure program books are up to date and uploaded.
  • Provide appropriate supervision levels per LOS.
  • Engage in all program services including toileting and behavioral interventions.
  • Report incidents immediately and assist with paperwork.
  • Assist coordinator with safeguards and notifications for incidents.
  • Ensure occurrence reports are reviewed and shared with the nurse the same day.
  • Ensure equipment and supplies are safe and properly stored.
  • Oversee cleaning and report hazards.
  • Undertake additional tasks as assigned.
  • Meet DSP Core Competencies by end of 1st year.
  • Maintain certifications AMAP, CPR/First Aid and SCIP as required.

Job description

Initial offers are made at the hourly base rate of $24.50. Additional compensation may be available after hire based on factors such as driving responsibilities, required certifications (e.g., AMAP), and length of time in the role. Pay increases within the range are based on certifications, training completion, and job performance. Hourly rate is only one part of Heartshare’s total compensation package, which may also include a bi-annual discretionary bonus.

The Sr Day Support Professional is responsible for writing the Staff Action Plan and ensuring the implementation of person-centered services for each individual. This includes ensuring implementation of the individual’s services, goals, and outcomes in relation to program-based and community inclusion activities. In addition, the Sr Day Support Professional is directly responsible for the individual’s overall physical and emotional well-being.

Primary Responsibilities And Essential Functions Of Position
  • Oversee DSP implementation of individuals’ plans, consistent with their specific services, goals, and outcomes. Participate in the planning and implementation of activities, both at the program and in the community, that address these goals and outcomes.
  • Promote choice and self-determination to support individuals to direct the course of their own lives. Ensure that services and goals reflect what the individuals want for themselves.
  • Ensure DSPs support and teach individuals according to their services, goals, and outcomes. Actively engage individuals in skills development, socialization, and task participation across all settings. Ensure that supports are provided with consideration for each person’s individualized needs and routines.
  • Promote and support the inclusion of individuals in their community according to their unique needs, interests, and desires. Model and encourage interaction between individuals and community members. Foster individuals’ personal growth by assisting in forming and maintaining friendships with neighbors, co-workers, and peers.
  • Assist in maintaining positive communications with family members, advocates, and service providers as needed. Act as advocate for individuals.
  • Transport individuals utilizing agency vehicles or assist them in taking public transportation, as required.
  • Plan monthly schedule with team for community outings and in-house activities. Be prepared each day to go into the community with individuals and oversee the DSPs in the community.
  • Participate in program planning meetings.
  • Create staff action plan with the team, put plan into review in Precision Care, and submit to supervisor for approval.
  • Review meeting notes and life plan to ensure the goals are consistent.
  • Once meeting is held (every 6 months) and plan is approved, send copy of the staff action plan to care manager, residence, and family within the required timeframe.
  • Upload all meeting documents, including sign-in sheet, meeting minutes, and proof of plan dissemination, into Precision Care.
  • Train all staff on plan and document training.
  • Document and ensure documentation of necessary information and data in areas such as medical, behavioral, individualized services, goals and outcomes, fire safety, community inclusion, communication books, etc., ensuring their accuracy and completeness.
  • Complete all monthly summaries by the 10th of every month.
  • Ensure all required documents in individuals’ program books are up to date and uploaded into Precision Care.
  • Provide appropriate levels of supervision on an individualized basis for staff and individuals, ensuring staff are trained on the LOS for each person.
  • Actively engage in all program services as needed, including toileting, behavioral interventions, community inclusion, emergencies, physical plant cleanliness, etc., as needed and required.
  • Report all incidents to program management immediately upon discovery. Immediately safeguard all individuals and assist with the completion of paperwork as required.
  • Assist program coordinator in securing safeguards, protective measures, and notifications for incidents.
  • Ensure all occurrence reports are reviewed, completed accurately, and given to the program nurse or coordinator the same day as the occurrence.
  • Ensure that equipment and supplies are appropriate, in good condition, and safely stored.
  • Oversee cleaning of program area and ensure cleaning is completed as frequently and thoroughly as necessary to meet cleanliness standards. Report any hazards or unsafe conditions immediately to supervisor.
  • Undertake any additional tasks and initiatives within the scope of the role as per the specific needs of the programs and individuals and as assigned by supervisor.
  • Meet or exceed the DSP Core Competencies Standards by the end of the 1st year of employment.
  • Maintain certification in AMAP, CPR/First Aid and SCIP, as required.
Qualifications
  • High School Diploma or GED required; bachelor's degree from four-year college or equivalent preferred.
  • Valid NYC Driver’s License required.
  • Minimum 2 years in the field with experience in supervision and paperwork.
  • Minimum of 2 years or more experience working with individuals with developmental disabilities.
  • Prior staff supervision experience strongly preferred.
