Job summary

Inova Fairfax Hospital is seeking a dedicated Clinical Pharmacist - Cardiology to join the team. This full-time role operates Monday through Friday with days and evenings rotating, and every third weekend, offering a sign-on bonus and relocation assistance.

The Pharmacist will interpret orders, provide clinical consultations, monitor therapies, manage formulary processes, supervise staff, and support continuous quality improvement while collaborating with cardiology and pharmacy teams to

Qualifications

• PharmD or B.Pharm required.
• Minimum 1 year hospital pharmacist experience.
• Licensed pharmacist in VA.
• BLS certification within 90 days of hire.

Responsibilities

• Interprets, prepares, and verifies medication orders.
• Provides clinical consultation and cost-effective alternatives.
• Issues controlled substances in compliance with DEA and VA Board of Pharmacy.
• Participates in formulary review and approval process.
• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Supervises pharmacy support staff.
• Participates in continuous quality improvement.
• Participates in decentralized rounding with clinical teams; responds to bedside alerts; precepts learners.

Inova Fairfax Hospital is seeking a dedicated Clinical Pharmacist - Cardiology to join the team. This role will be full-time Monday - Friday; Days/evenings rotating shift. Every 3rd weekend. |Sign-on Bonus & relocation assistance eligible.

Clinical Pharmacist Job Responsibilities

• Participates in the medication formulary review and approval process.
• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Supervises pharmacy support staff.
• Participates in continuous quality improvement.
• May perform additional duties as assigned, including, but not limited to:
  • Participating in decentralized rounding with clinical teams.
  • Responding to emergent bedside alerts and codes (adult and pediatric as designated).
  • Precepting learners as appropriate.

Minimum Qualifications

• Education: Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy.
• Experience: One (1) year working in a hospital as a pharmacist.
• Licensed Pharmacist in the state of VA.
• Certification: Basic Life Support (BLS) within 90 days of hire.

Preferred Qualifications

• PGY 1 and/or PGY 2 cardiology residency experience.
