ACMH invites applications for a Senior Case Manager/Case Manager position in Queens, NY. The role focuses on case coordination and delivering community rehabilitation services, with supervision by the Program Director.
Schedule is Monday–Friday, 9:00am–5:00pm, at the Queens Treatment Apartment site, with responsibilities spanning home visits, housing support, and crisis intervention. The ideal candidate holds an MA or BA in psychology or a related field, plus several years of direct care

Function: Provides case coordination and delivers community rehabilitation services.
Reports to: Program Director
Location: Queens
Schedule: Mondays through Fridays (9:00am-5:00pm)
Sr. Case Manager Qualifications: M.A. in psychology or a human service related field. B.A. in psychology or a human service related field with a minimum of 3 years of experience providing direct care in human services. Experience working with adults with serious mental illness preferred. Strong written and verbal communication skills. Master’s Degree preferred.
Scattered Site SCM Salary: $63,905 plus generous benefits

Scattered Site CM Salary: $55,632 plus generous benefits
