ACMH is seeking a Senior Case Manager/Case Manager for the Queens Treatment Apartment (QTA). You will coordinate case management, deliver community rehabilitation services, and work with the Program Director in Queens.
The role involves home visits, plan development, and crisis intervention as part of a multidisciplinary team, with a focus on supporting adults with serious mental illness in access to services and housing stability.
Function: Provides case coordination and delivers community rehabilitation services.
Reports to: Program Director
Location: Queens
Schedule: Mondays through Fridays (9:00am-5:00pm)
Sr. Case Manager Qualifications: M.A. in psychology or a human service related field. B.A. in psychology or a human service related field with a minimum of 3 years of experience providing direct care in human services. Experience working with adults with serious mental illness preferred. Strong written and verbal communication skills. Master’s Degree preferred.
Scattered Site SCM Salary: $63,905 plus generous benefits
