Position: Spiritual Caregiver - Home Health & Hospice

Join CHI Home Health and Hospice to provide compassionate, personalized care in patients' homes, focusing on end-of-life care for those with a life-limiting diagnosis. The Spiritual Caregiver is an essential part of our team, supporting patients and families through their life journey.

Responsibilities

• Provide spiritual and bereavement care to hospice clients, families, staff, and volunteers under the supervision of the agency Director.
• Document assessments, care plans, and interventions in the electronic medical record.
• Complete spiritual assessments within five days of admission.
• Visit patients per the care plan and adjust frequency as patient needs change.
• Offer spiritual and emotional support, coordinate with community clergy, and manage bereavement services and follow-up assessments.
• Develop and document bereavement care plans, provide grief counseling, and coordinate support groups and memorial events.
• Facilitate in-services, education, and community events related to hospice care.

Qualifications

• Appropriate education for a spiritual care provider and experience in bereavement support.
• Completion of pastoral care and bereavement training programs.
• CPR certification and valid driver’s license with insurance coverage.

Preferred Skills

• Knowledge of spiritual assessment and intervention techniques.
• Excellent communication skills, both verbal and written.

Pay Range

$17.93 - $24.66 per hour
