Job summary

The Spine Nurse Navigator at EvergreenHealth in Kirkland, WA enhances patient experience by interfacing with care teams and guiding Spine Center patients from admission to follow-up. The role acts as a patient-centered resource, educating and assisting with access to services, while serving as a consistent contact for families post-discharge.

Orientation covers orthopedic and spine care pathways; staff participate in ongoing education and program development to improve spine care delivery.

Qualifications

• Bachelor’s in Nursing (BSN) required.
• Licensed as a Registered Nurse in WA or holding NLC license.
• Minimum of 3 years full-time healthcare experience.
• Experience with direct customer service.
• Computer skills.

Responsibilities

• Facilitate exemplary patient care and manage Spine service policies and standards.
• Communicate with Spine Program physician(s) and providers to schedule services.
• Develop and foster comprehensive Spine Surgery Program initiatives.
• Coordinate care among nursing, physicians, therapists, pharmacy, and case management.
• Maintain documentation of nurse navigation activities for each patient.
