Vanderbilt Spine Center in Nashville, TN is seeking a Patient Care Programs Coordinator for the Spine Clinic to coordinate and support patient care programs across the care journey. The role interfaces with clinical staff to ensure program goals, regulatory standards, and patient access needs are met in a fast-paced academic health system.

The position supports patients and families from referral through follow-up, with a focus on coordinating diagnostics, treatments, surgeries, and ongoing care

Qualifications

• Coordinates and supports individual patients and families during care program(s).
• Supports program goals with guidance to ensure successful outcomes.
• Provides access to providers and educational materials for patients.

Responsabilités

• Partners with clinical care staff and providers to ensure regulatory requirements are met to maintain patient care needs, program guidelines and accreditation standards.
• Acts as a point of access for patients, referring providers and the Vanderbilt community.
• Develops short and long term goals to ensure programs' success.
• Provides support to patient care team to market programs and streamline business models to allow for appropriate growth.
• Establishes contact with existing, newly diagnosed, potential, or referred patients by facilitating access to providers, educational materials and communication with care providers.
