Qualifications

Responsibilities

• Assess new patients referred to spine specialists prior to first visit to identify support needs.
• Document patient assessments and interventions in the EMR.
• Refer patients to navigation services and coordinate with multidisciplinary teams.
• Provide education and emotional support through intake and treatment planning.
• Schedule patients with appropriate providers at correct locations and times.
• Coordinate needed diagnostics prior to first visit to expedite treatment start.
• Develop relationships with referring physicians and ensure follow-up communications.
