Job summary

Aura Care Group invites a Speech Therapist (SLP) to evaluate, diagnose, and treat communication, cognitive, and swallowing disorders. The position aims to help residents improve speech, language, voice, memory, and safe swallowing to maximize independence and quality of life in a skilled nursing setting.

You will develop individualized treatment plans, document evaluations and progress, and collaborate with physicians, nursing staff, and other therapy disciplines.

Qualifications

  • Current Minnesota SLP license or eligibility.
  • Master's degree in Speech-Language Pathology from an accredited program.
  • CCC-SLP credential preferred (ASHA).
  • Strong assessment, treatment, and documentation skills.
  • Excellent communication and interpersonal skills.
  • Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

  • Evaluate residents for communication, cognitive, and swallowing needs.
  • Develop and implement individualized treatment plans based on assessments and physician orders.
  • Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety.
  • Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards.
  • Educate residents, families, and staff on communication strategies, swallowing precautions, and home programs.
  • Collaborate with physicians, nursing, and other therapy disciplines to ensure coordinated care.
  • Participate in interdisciplinary care plan meetings and quality improvement programs.
  • Maintain professional licensure and adhere to ethical practice standards.

Position Summary:

Key Responsibilities:
  • Evaluate residents/patients for communication, cognitive, and swallowing needs

  • Develop and implement individualized treatment plans based on assessments and physician orders

  • Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety

  • Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards
