Job summary

Proforma Health seeks a licensed Speech Language Pathologist (SLP) to provide home health care in Tyler, TX. The role focuses on restoring communication, cognition, and swallowing functions in patients’ homes.

You will evaluate patients, develop and implement personalized treatment plans, and deliver evidence-based therapy, while coordinating with an interdisciplinary team and educating families. Documentation and physician communication are essential.

Qualifications

  • Master's or Doctoral degree in Speech-Language Pathology.
  • Current licensure to practice Speech-Language Pathology.
  • Minimum 1 year of clinical experience (home health preferred).
  • CPR certification required.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Conduct patient evaluations in home health settings.
