Job summary

Nationwide Healthcare Services is seeking clinicians to join our rehabilitation departments across Delaware, Montgomery and Philadelphia counties. We have Full-Time, Part-Time and PRN positions available.

Under a physician’s order, clinicians will deliver direct resident care per an established plan of care with various treatment modalities. Current state licensure and SNF experience preferred. We emphasize compassionate care, privacy compliance and ongoing learning across the team.

Qualifications

• Graduated from an accredited program approved by their professional association.
• Current licensure by the applicable state agency.

Responsibilities

• Evaluates residents and develops appropriate care plans.
• Monitor residents' progress and adjust treatments accordingly.
• Provide supervision for Occupational Therapy/Physical Therapist Assistants, students and support personnel in accordance with state license requirements and professional standards.
• Under a physician’s order, provides direct resident treatment in accordance with an established plan of care.
• Documentation of services provided within Rehab Optima on a timely basis.
• Demonstrates knowledge of Medicare and other reimbursement regulations.
• Contributes to team meetings and quality improvement activities.
