First Steps Nursing and Therapy is seeking a licensed Speech Language Pathologist to provide in-home therapy for children in San Antonio, Texas and surrounding areas.

The therapist will conduct evaluations, develop treatment plans, document progress, and coordinate visits with families, physicians and caregiving teams.

Qualifications

• Texas licensure as a Speech Language Pathologist.
• One year of work experience preferred.
• Pediatric and home health experience preferred.
• Excellent communication and interpersonal skills.
• Familiarity with Kantime and EVV.

Responsabilités

• Conduct evaluations and complete evaluation reports.
• Plan activities and provide therapy services.
• Record treatment and patient response weekly.
• Maintain patient records and summary reports.
• Coordinate visits with Therapy Coordinator and other personnel.

First Steps Nursing and Therapy is looking to hire registered Speech Language Pathologist to provide in home therapy services to children in San Antonio, Texas and the surrounding areas.

Job Summary

The therapist is a qualified professional person who directs, supervises, evaluates and provides therapy services to patients in the home as prescribed by the attending physician.

Qualifications

• Licensure or certification by the state of Texas as a Certified Speech Language Pathologist

• One (1) full year of work experience preferred

• Pediatric and home health experience is preferred (not required)

• Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.

• Familiarity with Kantime and EVV

Responsibilities

• Conduct evaluations and complete evaluation reports in timely manner

• Plan activities and provide occupational therapy services

• Record type of treatment and patient’s reaction to it on clinical/progress notes and incorporated in the patient’s record weekly.

• Maintain adequate records on all patient’s, including summary reports

• Utilize community resources and Agency personnel for proper referral

• Attend case conferences as requested

• Participate in staff development activities and in-service education as requested

• Confirm, on a weekly basis, the scheduling of visits with the Therapy Coordinator to coordinate necessary visits with other personnel

• Participate with staff, patient, and physician in discharge planning activities
