First Steps Nursing and Therapy is looking to hire registered Physical Therapist to provide in home therapy services to children in San Antonio, Texas and the surrounding areas.

Job Summary:

The therapist is a qualified professional person who directs, supervises, evaluates and provides therapy services to patients in the home as prescribed by the attending physician.

Qualifications:

• Licensure or certification by the state of Texas as a Physical Therapist
• One (1) full year of work experience preferred
• Pediatric and home health experience is preferred (not required)
• Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
• Familiarity with Kantime and EVV

Responsibilities :

• Conduct evaluations and complete evaluation reports in timely manner
• Plan activities and provide therapy services
• Record type of treatment and patient’s reaction to it on clinical/progress notes and incorporated in the patient’s record weekly.
• Maintain adequate records on all patient’s, including summary reports
• Utilize community resources and Agency personnel for proper referral
• Attend case conferences as requested
• Participate in staff development activities and in-service education as requested
• Confirm, on a weekly basis, the scheduling of visits with the Therapy Coordinator to coordinate necessary visits with other personnel
• Participate with staff, patient, and physician in discharge planning activities
