Job summary

Connect Pediatrics is seeking a Pediatric Speech-Language Pathologist to join our team serving the San Antonio, Texas area. This role delivers in-home therapy to medically fragile and special needs children, building strong relationships with families and coordinating with a multidisciplinary team.

You will perform comprehensive assessments, develop customized treatment plans, document progress, and provide family training. A Texas license and BLS/CPR are required.

Qualifications

• Active, valid Texas Speech-Language Pathology license.
• Current BLS/CPR certification.
• Reliable transportation and valid driver's license.

Responsibilities

• Perform comprehensive pediatric evaluations using standardized tests.
• Develop and implement treatment plans guided by physician orders.
• Provide in-home therapy to pediatric patients with various diagnoses.
• Maintain regular communication with families on progress and goals.
• Collaborate with a multidisciplinary team to create holistic plans.
• Document care thoroughly and share updates with the healthcare team.
• Participate in team meetings to refine treatment strategies.

Responsibilities

Perform comprehensive evaluations, using standardized tests to determine pediatric patients' needs for therapy.

Use your clinical judgment to develop and implement relevant treatment plans aligned with physician's orders.

Provide in-home therapy services for pediatric patients with a wide range of diagnoses.

Engage with patients' families, providing them with regular communication and updates on goals for their child.

Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient

Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.

Actively participate in one-on-one meetings with team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
