Job summary

PureHealth is seeking a Speech-Language Pathologist to provide therapy services in a post-acute setting, in compliance with laws and our Corporate Compliance Program. The clinician will evaluate, diagnose, and treat swallowing, speech and language impairments with advanced clinical reasoning and family training.

You will design and modify interventions, document plans of care and progress notes, participate in rounds, and collaborate with an interdisciplinary team.

Qualifications

• Master’s Degree from an accredited program.
• Valid State SLP License.
• Minimum one year of related experience.

Responsibilities

• Provide direct patient care meeting department and professional standards, including evaluation and treatment planning.
• Design, implements, and modifies interventions to address swallowing, speech and language impairments.
• Complete documentation: evaluation plans, progress notes, discharge summaries, and billing.
• Discharge patients with adequate discharge planning including family training.
• Maintain assigned caseload aligned with productivity standards.
• Participate in interdisciplinary care planning, rounds, and care conferences.
