Job Summary

The Speech-Language Pathologist (SLP) evaluates, diagnoses, and treats residents with speech, language, voice, cognitive-communication, and swallowing disorders. The SLP develops individualized treatment plans to enhance residents' communication abilities and overall quality of life. Under the guidance of our Director of Rehabilitation, you'll perform essential resident tasks, helping us uphold our reputation for exceptional care and innovation in skilled nursing and rehabilitation.

Principle Duties and Responsibilities (*Essential Functions):

These duties and responsibilities are essential for maintaining quality resident care and ensuring smooth operations at Dwyer Home.

Assessment and Diagnosis:

Conduct comprehensive evaluations of residents' speech, language, cognitive-communication, and swallowing abilities.

Use standardized tests, clinical observations, and resident history to identify areas of difficulty.

Treatment Planning and Implementation:

Develop and implement individualized treatment plans tailored to each resident’s needs.

Use evidence-based techniques to improve speech, language, voice, and swallowing functions.

Collaborate with other healthcare professionals to address underlying conditions affecting communication or swallowing.

Resident and Family Education:

Educate residents and caregivers about therapy goals, progress, and strategies to improve communication and swallowing.

Provide training in the use of augmentative and alternative communication (AAC) devices if applicable.

Documentation and Reporting:

Maintain accurate and detailed records of evaluations, treatment plans, and resident progress.

Prepare and submit reports to physicians, insurers, and interdisciplinary teams as needed.

Work closely with Physical Therapists, Occupational Therapists, and other professionals to provide comprehensive care.

Participate in care team meetings and contribute to treatment plan adjustments.

Professional Development:

Stay current with advancements in speech-language pathology, including research, technologies, and best practices.

Maintain state licensure and certifications through continuing education.

May perform other duties as assigned.

Minimum Qualifications:

Education and Certification:

Master's degree in Speech-Language Pathology from an accredited program.

Licensure/Certification : State licensure and Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA (required).

Successful completion of all Dwyer Home specified training programs.

Proficiency in reading and comprehending simple instructions, short correspondence, and memos.

Ability to write simple correspondence.

Effective presentation skills can convey information to residents and other employees in one‑on‑one and small‑group situations.

Proficiency in basic mathematical operations, including addition, subtraction, multiplication, and division, using whole numbers, common fractions, and decimals.

Capacity to apply common sense understanding to execute detailed but uninvolved written or oral instructions.

Ability to handle problems involving a few concrete variables in standardized situations.
