Job description

A skilled nursing facility in the West Allis area is seeking a compassionate and motivated Speech Language Pathologist (SLP) to join our rehabilitation team. This is an excellent opportunity to work alongside an experienced interdisciplinary team dedicated to helping residents improve communication, cognition, and swallowing function while enhancing their overall quality of life.

Responsibilities

• Provide speech therapy evaluations and develop individualized treatment plans
• Assess and treat speech, language, cognitive, communication, and swallowing disorders
• Implement evidence-based treatment interventions to maximize resident outcomes
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Maintain timely and accurate documentation in accordance with Medicare, state, and facility guidelines
• Participate in care planning meetings and discharge planning activities
• Educate residents, families, and staff regarding treatment plans and recommendations

Qualifications

• Master’s Degree in Speech Language Pathology required
• Current Wisconsin Speech Language Pathologist license required
• Certificate of Clinical Competence (CCC-SLP) preferred
• Experience in a skilled nursing, rehabilitation, or post-acute setting preferred
• Strong communication, organization, and documentation skills
• Passion for providing exceptional resident-centered care
