Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin, is seeking a dedicated Speech-Language Pathologist (SLP) on a per diem basis. You will evaluate and treat patients with communication and swallowing disorders, working with an interdisciplinary team to develop individualized plans and ensure high-quality care in our rehab facility.

We are seeking a dedicated and experienced Speech-Language Pathologist (SLP) to join our team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a Per Diem SLP, you will have the opportunity to work with a diverse range of patients and contribute to providing high-quality care in our rehabilitation and nursing facility.

About the Role:

As a Speech-Language Pathologist (SLP), you will be responsible for evaluating and treating patients with communication and swallowing disorders. Your expertise will help patients regain their ability to communicate effectively and safely manage their food and liquid intake. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans, ensuring that each patient receives the best possible care.

Responsibilities:
  • Evaluate patients with communication and swallowing disorders, including speech, language, and cognitive impairments
  • Develop and implement individualized treatment plans to improve patients' ability to communicate effectively and safely manage their food and liquid intake
  • Conduct speech therapy sessions, including individual and group sessions, to address patients' specific needs
  • Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists, to ensure comprehensive care
  • Communicate with patients, families, and caregivers to educate them on treatment plans and progress
  • Maintain accurate and detailed records of patient evaluations, treatment plans, and progress
