Position Summary

Reporting to the Clinical Director/Manager, the SLP provides speech‑language therapy services in patients' homes in accordance with New Hampshire licensure standards and physician orders. Responsibilities include assessment, treatment planning, intervention, patient education, documentation, and collaboration with the interdisciplinary care team.

Key Responsibilities

• Conduct patient evaluations and develop individualized plans of care
• Implement and adjust evidence‑based treatment plans
• Educate patients and caregivers on therapeutic strategies
• Complete timely and accurate clinical documentation
• Communicate with physicians and healthcare team members
• Participate in care conferences and interdisciplinary collaboration
• Support agency quality initiatives, educational programs, and committee work
• Safely travel throughout the Granite VNA service area

Requirements

1. Master's degree in speech‑language pathology, communications sciences and disorders, or communications disorders.
2. Graduate of an approved speech‑language program (ASHA) with a certificate of clinical competency in speech‑language pathology (CCC‑SLP), accompanied by current New Hampshire speech‑language pathology license.
3. Two years of inpatient/skilled/acute or community health speech‑language experience.
4. Valid driver's license, reliable transportation and an acceptable driving record.
