Job summary

Home Health of Western Colorado is seeking a Speech Therapist to provide one-on-one care in patients’ homes across Western Colorado. You will assess speech, language, cognitive-communication, and swallowing disorders, and develop individualized treatment plans.

You will educate patients and families, document progress, and coordinate care with physicians and other clinicians as part of an interdisciplinary team in a patient-centered, community-focused environment.
