Job summary

Home Health of Western Colorado is seeking a Speech Therapist to provide direct, one-on-one care in patients' homes across Western Colorado. You will assess speech, language, cognitive-communication, and swallowing disorders and develop personalized treatment plans.

You will educate patients and families, document progress, and coordinate care with physicians and other clinicians as part of a patient-centered, community-focused team that supports independence and broad clinical skill development

Qualifications

• Master's degree in Speech-Language Pathology.
• Colorado licensure or eligibility to obtain licensure.
• Strong communication and patient education skills.

Responsibilities

• Assess speech, language, cognitive-communication, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Educate patients and families on conditions and progress.
