Job summary

The Palms Home Care in Fort Myers, FL is seeking a compassionate Speech-Language Pathologist to provide home health services to adult and geriatric patients throughout the Fort Myers area.

This role welcomes experienced home health SLPs and clinicians transitioning from pediatric settings into adult and geriatric care, offering meaningful work helping patients regain independence in the comfort of their homes.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Active Florida SLP license required.
• CCC from ASHA preferred.
• Home health or geriatric experience preferred but not required.
• Pediatric SLPs transitioning to adult/geriatric care encouraged.
• Strong clinical assessment, communication, and organizational skills.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Complete comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop and implement plans of care based on patient needs.
• Provide skilled therapy to patients recovering from strokes or neurological conditions.
• Treat speech, language, cognitive, and swallowing impairments.
• Educate patients and families on treatment plans and home exercises.
• Collaborate with physicians, nurses, and therapists.
• Communicate changes in condition to physicians and leadership.
• Complete timely clinical documentation per regulatory requirements.
• Manage an average caseload of about six patient visits per day.

Job description

The Palms Home Care is seeking a compassionate and motivated Speech-Language Pathologist to provide home health services to adult and geriatric patients throughout the Fort Myers area.

We welcome experienced home health SLPs, as well as clinicians from pediatric settings who are interested in transitioning into adult and geriatric care. This is a rewarding opportunity to explore a new area of speech therapy while helping patients regain independence and improve their quality of life in the comfort of their homes.

Responsibilities

• Complete comprehensive speech, language, cognitive, communication, voice, and swallowing evaluations.

• Develop and implement individualized plans of care based on each patient’s needs and goals.

• Provide skilled therapy to patients recovering from strokes, neurological conditions, surgery, illness, injury, or hospitalization.

• Treat patients experiencing speech and language impairments, cognitive or memory changes, and swallowing difficulties.

• Educate patients, families, and caregivers on treatment plans, safety strategies, and home exercises.

• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.

• Communicate changes in condition and make recommendations to physicians and clinical leadership.

• Complete timely and accurate clinical documentation in accordance with agency and regulatory requirements.

• Manage an average caseload of approximately six patient visits per day.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.

• Active and unrestricted Florida SLP license.

• Certificate of Clinical Competence from ASHA preferred.

• Home health or geriatric experience is preferred but not required.

• Pediatric SLPs interested in transitioning to adult and geriatric care are encouraged to apply.

• Strong clinical assessment, communication, and organizational skills.

• Reliable transportation, valid driver’s license, and current automobile insurance.
