Job summary

Concierge Home Care in Clearwater, FL is seeking a licensed Speech-Language Pathologist to deliver home health therapy with emphasis on communication, cognition, and swallowing. You will plan and execute one-on-one patient care in patients' homes with autonomy and clinical leadership.

Qualifications

Your Role as a Home Health Speech-Language Pathologist

As a Home Health SLP, you will provide skilled speech therapy services in the comfort of patients’ homes while helping them improve communication, cognition, and swallowing function.

Responsibilities include:

• Conduct comprehensive speech, language, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on communication, cognition, and swallowing disorders
• Educate patients and caregivers on techniques to improve functional outcomes
• Collaborate with physicians and interdisciplinary care teams
• Complete Medicare OASIS documentation as required
• Document visits efficiently using AI-assisted tools
