Job summary

Concordia Visiting Nurses, part of Concordia Lutheran Ministries, seeks a Speech-Language Pathologist to provide in-home therapy for adults and geriatrics in Western Pennsylvania. You will assess, treat, and educate patients and families, developing personalized plans and coordinating with the care team.

The role offers flexible scheduling, autonomy with clinical support, and a strong mission to deliver compassionate care in patients’ homes.

Qualifications

• Current license as a Speech-Language Pathologist in PA.
• ASHA CCC-SLP certification preferred.
• At least 1 year of home health experience is a plus.

Responsibilities

• Provide skilled speech therapy services to patients in their homes.
• Develop and implement personalized care plans.
• Evaluate speech, language, cognition, voice, and swallowing disorders.
• Educate patients and caregivers on therapeutic strategies.
• Collaborate with nurses, PT/OTs, and physicians to ensure the best outcomes.

Who you are as a Speech Therapist ST:

• Current license as a Speech-Language Pathologist in (PA)
• ASHA CCC-SLP Certification
• 1+ years of experience (home health a plus but not required—we’ll train the right candidate!)
• Strong interpersonal and communication skills
• Reliable transportation and valid driver’s license
